Per the No Surprises Act, Valley Forge & Bolt Mfg. Co. is required, as a self-funded medical group, to publish machine-readable files on a public site. These files linked below follow the Centers for Medicare & Medicaid Services (CMS) guidelines to post negotiated rates for in-network providers and historically allowed amounts for out-of-network providers.
